I authorise the South Pacific Division of the Seventh-day Adventist Church to obtain information from Avondale University for the purpose of assessing my application and suitability for employment. This information may include my completed application form, academic transcript/s and photograph.

I authorise any prospective Seventh-day Adventist Church employer to contact any interview panel member I have been interviewed by and authorise that panel member to provide information relating to my application for employment to a Seventh-day Adventist Church entity or institution within or outside Australia.

I authorise any interview panel member I have been interviewed by and any prospective employer to contact any member of the Avondale University teaching staff to obtain relevant information as it relates to my suitability for employment.

I authorise any interview panel member and any prospective employer to contact any referee nominated by me in order to obtain relevant information as it relates to my suitability for employment.
